Regular update: added/edited 2200+ records

We have released a regular update - version 9.4. Changes:
- Updated about 1900 Visa/MC/Maestro records with new issuer, card type, card category, URL and phone information;
- Updated about 450 records with new country information;
- Updated about 3000 US banks with new URLs and phone numbers;
- Added a few new records and removed decommissioned ones.
